The BCMA Emergency Medical Card records the name and contact information of your physician and next of kin.

In the event of an emergency, it is helpful to have the information to help treat your medical needs - such as your physician's name, your next of kin, and any medical conditions or medications - in a place that is easy or emergency medical services to find.

  1. Open and print the emergency medical card.

  2. Fill in your information

  3. Cut out the card along the outside dotted line

  4. Fold over the two-sided card along the middle dotted line

  5. Keep this card in your wallet
